Make a Simple PHP Mailer Script - More complex script here
After you submit the form, just copy the script into a notepad window and save it as a .php file.  You can then load it into your favorite editor and make it look the way you want.

1. The email address the form will send to
Address that will RECEIVE emails sent by your form.

2. The Subject of the Emails
(Leave Blank for user-defined subject)

3. Now select fields the user will fill in on the mail-sending page (Suggested to leave all selected)
 Name  Email Address
Message/Comments
